HOP program is the nation’s first comprehensive program to test and evaluate the impact of providing select evidence-based, non-medical interventions related to housing, food, transportation and interpersonal safety and toxic stress to Medicaid Managed Care enrollees.

Do You Qualify?
To qualify for Healthy Opportunities Pilot services, Medicaid Managed Care enrollees in Standard Plans must live in a Pilot Region and have qualifying physical/behavioral health criteria and at least one social risk factor.

Persons who have dual (Medicare/Medicaid)
