Access East has partnered with Mount Carmel HELPS, Inc as part of North Carolina’s Healthy Opportunities Pilot Program to provide the following services:

• Housing Services

• Interpersonal Violence/Toxic Stress

• Transportation Services

Am I Eligible?

To be eligible for the Healthy Opportunities Pilot program an individual must have Medicaid AND be a Medicaid Managed Care Member. If you meet both of these criteria, please contact Member Services on the back of your insurance card.  

Eligibility Requirements​

1. Client must have at least One Physical or Behavioral Health Condition

• Adults (e.g., 2 or more chronic conditions)

• Pregnant Women ‍(e.g., multifetal gestation)

• Children, ages 0-3 ‍(e.g., Neonatal intensive care unit)

• Children 0-21 ‍(e.g., Experiencing 3 or more categories of)

​2. At least One Social Risk Factor

• Homeless/housing insecurity

• Food Insecure

• Transportation Insecure

3. Must live in one of the following Pilot Regions

• Beaufort

• Bertie

• Chowan

• Edgecombe

• Halifax

• Hertford

• Martin

• Northampton

• Pitt
